2011 Registration Form
All pertinent information concerning the health and safety of your child is listed on this form. Please complete a separate form for each child. Due with registration fee and any other applicable forms.

Weekly/Daily Enrollment Form
Complete this form if you are signing up by the day or week. You may complete additional forms throughout the summer to add more days/weeks.

YWCA Swim Form (Preschool Only)
The Gateway YWCA requires parent permission for children to participate in swim lessons. Preschool parents only must complete this form. Due with registration.

Medical Form
All new camp participants must turn in a Physician's signed medical report. If there is not a report on file for past participants or current students, a new form is required. All participants must have an updated immunization record on file as well. Due with registration.

Medication Administration Form
If medication is medically necessary for your child such as severe allergies, dr. prescribed daily maintenance medication, asthma, etc., please complete this form and place in a zip loc bag along with the medication. The medication must be in its original container. See the Camp Falcon Handbook for more info. on medication policies.

Summary of NC Childcare Law
Camp Falcon falls under St. John's childcare license with the state of NC. All parents are required to receive and read a summary of childcare law.
